We’re on 27th Ave. Just south of the 101 Freeway. Right across the street from the Honor Deer Valley Hospital.

+ Is Dr. Bridge’s office, Honor Health ENT?

No. Honor Health no longer has an ENT specialty clinic. Dr. Bridge is independent now. We are still contracted with most of the honor health network plans. We are on a different system, but we can still access most of their records.

+ How do I get a disk from a study?

Call the radiology facility that you had the study done at and they will prepare a disk for you to pick up.

+ What do I need to bring to my appointment?

• Please bring you insurance card and photo ID. Even if you have registered online.
• An up to date medication and allergy list
• Copies of any radiology studies (CT Scan, MRI, Ultrasound) you have had of any of the head and/or neck areas in the last year.
• If at all possible, obtain a disk of the above mentioned radiology studies.

+ Are you contracted with my insurance?

• We are contracted with most major commercial plans, Medicare, and most Medicare replacement plans.
• The only AHCCCS plan through the state that we are contracted with is Mercy Care.
• We are NOT contracted with Cigna Healthsprings or United Healthcare Banner Network.
• We try to keep all our contracted plans up to date but always recommend that you check with your health plan to figure out if we are in network with yours.
